|RN/LPN OCCASSIONAL STATUS
JOB OBJECTIVES :
Under the general supervision of the ADON or DON or Supervisor, performs nursing services to residents consistent with licensing requirement and based on individualized resident care plans, physician orders and according to policies and procedures established by Pine Crest Nursing Home. This position carries withit the responsibility of providing input into resident care decisions and policies and directs supervision of CNA’s and other paraprofessional nursing staff.
JOB STANDARDS :
•Graduation from an accredited school of nursing and currently licensed to practice nursing in the State of Wisconsin. Current CPR certification is required.
•Must have considerable knowledge of professional theory and practices and the ability to function independently and in a leadership capacity within assigned area is required.
•Must have the ability to make accurate observations, organize work, set priorities and establish routines for self and others. Must be able to respond and act in a leadership capacity in demanding and emergency situations.
•Previous geriatric nursing experience is preferred.
•Must have the ability to communicate in English, both verbally and in writing including the ability to transcribe physician orders completely and precisely. Must have basic math skills. Must be able to write and orally present clear and accurate reports and objectively evaluate and direct the work of Certified Nursing Assistants.
•Must have considerable knowledge of and the ability to use aseptic technique, a thorough understanding of infection control procedures and theories and an understanding of Universal Precautions.
•Must be honest, dependable and able to exhibit a caring manner toward residents, form therapeutic and professional relationships with residents and communicate effectively with residents, staff physicians, family members, other staff and the Supervisor.
•Must be able to establish effective working relationships with the RN Supervisor and DON which includes open communication and the provision of ongoing reports as to the functioning of the unit
•Must be able to maintain entrusted confidential information appropriately.
•Must adhere to departmental dress code.

ILLUSTRATIVE EXAMPLES OF ESSENTIAL JOB FUNCTIONS :
•Directs and assists in the provision of direct resident care in accordance with HSS 132 and the policies of Pine Crest Nursing Home.
•Observes, assesses and reports changes in resident conditions to physicians and responsible family members.
•Accepts verbal, written and telephone orders from physicians and transcribes them according to procedure
•Coordinates physician orders with appointments and notifies other departments of changes affecting them. (Diet changes, PT, OT, ST referrals, transportation and consultants).
•Monitors changes in resident lab values, appetite, effectiveness of scheduled medications, procedures or changes in the cares delivered, updates CNA Care Cards as needed.
•Oversees resident care delivered by CNAs and Restorative Nursing.
•Updates CNAs on resident changes and directs the institution of new procedures needed.
•Completes assessments for all new residents assigned to the unit according to policy.
•Prepares, administers and documents medications as ordered by the physician.
•Provides nursing treatments to residents as ordered by the physician.
•Participates in care planning sessions for individual residents, updates and keeps current MDS and insures that resident care plans are updated and current.
•Provides appropriate documentation in resident medical records in a precise and accurate manner according to policy.
•Assists physicians when “rounds “are made if necessary.
•Communicates effectively with physicians regarding observations and assessments made.
•Educates residents and/or responsible family members of medications received by the resident, notifying them of side effects and desired effects or outcomes.
•Assists in meeting the emotional needs of residents and family members.
•Supports resident rights and forms therapeutic relationships with residents and family members.
•Confers with representatives from other departments (Dietary, Activity, Social Services) regarding specific problem areas for residents and assists in problem solving.
•Operates all nursing equipment in accordance with Pine Crest policies.
•Performs other related duties as assigned by the RN Supervisor or DON.
